Overview: The MDS Coordinator RN conducts and coordinates the comprehensive assessment of long-term care residents, which includes the MDS, RAPS and interdisciplinary assessment for initial, quarterly and significant changes. The MDS Coordinator assures the accuracy of the MDS through communication and documentation with staff. Coordinates the completion and accuracy of the individual resident’s care plan, the Medicare A coverage, and documentation guidelines assuring compliance.
Key Duties & Responsibilities
People: Schedules care conferences and invite families/residents to attend; determines the reason for Med A coverage, and coordinates with therapy office and nursing; coordinates care plan development for accuracy, completion, and attendance by all disciplines.
Quality: Prepares Quality indicator summaries, and Quality Measures summaries.
Operational Excellence: Schedules, tracks, and assures completion of MDS and documentation timely, submits complete and accurate MDS to State agencies, conducts triple check on Med A claims in conjunction with office and medical records, and follows safety, infection control, and confidentiality standards.
Schedule: Approximately 40 hours per week, day shift, and will participate in on-call rotation.
