Position Overview: As a resident advocate, counsels and assists nursing center residents and families needing assistance. Contacts appropriate agencies such as ombudsman, adult protection agency, psychiatrists and psychologists, as necessary. Provides interventions to meet psychosocial needs of residents and works with the Nursing Center team in setting resident goals. Understands the Patient Bill of Rights and ensures rights are not violated.
Key Duties & Responsibilities
People: Provides consultation to staff, community agencies, etc., in efforts to solve the needs and problems of the residents. Assists residents with problems such as personal and family adjustments, finances, food, clothing, and physical and mental impairments. Provides information to residents and families on advanced directives, living wills, and limited medical treatment. Investigates allegations of abuse, documents findings and recommends action to be taken.
Quality: Assists with discharge planning in order to ensure a smooth transition to home. Performs and documents quarterly and annual assessments. Secures information about medical, psychological, and social factors contributing to resident’s situation and evaluates. Works with medical and nursing teams regarding appropriate use of psychotropic medications and documentation of behavioral patterns. Participates in care conferences as social services member of interdisciplinary care team. Recommends interventions of other health professionals such as nursing or physician consults.
Operational Excellence: Keeps abreast of current Federal and State Regulations, as well as professional standards and makes recommendations for changes in policies and procedures. Coordinates admissions in absence of Admissions Director.

Schedule: Full-time, 40 hours per week, generally Monday through Friday.
