Position Overview: The Certified Nursing Assistant (CNA) is responsible for direct care of nursing center residents/patients.  Typical  responsibilities include answering call lights, bells, or intercoms to determine resident needs, bathing, dressing and undressing residents, serving food, transporting residents, assisting residents to walk, changing bed linens, taking and recording temperature, blood pressure, pulse, and respiration, and providing direct nursing care according to all organizational policies and procedures.

Key Duties & Responsibilities:

People: The CNA will work as a member of our team ensuring the timely, compassionate, and appropriate care of residents.  He/she will communicate effectively with co-workers and the Nurse in Charge.

Quality: The CNA must adhere to all safety and procedural requirements in order to provide quality care.  He/she must follow the documented care plan, perform correct transfers of residents, understand and follow correct equipment operations and activities, provide ROM/positioning and ambulation of residents, and assist in providing the least restrictive environment for residents. Must maintain a Colorado CNA license.

Operational Excellence:  The CNA will participate in care planning, admissions and discharge processes, and orientation of new CNAs. The CNA will be dependable and reliable in attendance.   

Qualifications:

Must have current CNA license

Must be at least 18 years of age

Experience in long term care as a CNA is preferred

Experience with dementia is a plus

Schedule:  Full-time averages 32 – 37.5 hours per week.   Part-time averages 15 – 20 hours per week. Shifts are days (6 AM – 2 PM), evenings (2 PM – 10 PM), nights (10 PM – 6 AM) including some weekends.

Title RN – Night Shift – Full Time or Part Time

Position Overview: Under the direction of Nursing Center management, the RN is responsible for providing prescribed medical treatment and personal care services to ill, injured, convalescent, and disabled persons in the Nursing Center.  Typical responsibilities include administering medications, keeping accurate records, taking vitals, dressing wounds, giving enemas, collecting samples for analysis, assessing residents and responding to their changes/needs, updating care plans, and assisting residents with basic needs and comfort.  May make beds, bathe and feed residents, and supervise others.  Nurse shall provide direct nursing care according to all organizational policies and procedures.

Qualifications:

    • Minimum of two years experience in long term care
    • Ability to stand and walk frequently
    • Ability to lift or move up to 50 pounds and appropriately use mechanical lifts
    • Experience with dementia is a plus.

Schedule: Average 32 – 40 hours per week. Shift is 10:00 pm – 6:30 am, including every other weekend and some holidays. $2.00/hr weekday night differential $4/hr weekend night shift differential.

Assure the accuracy of the MDS through communication and documentation. Assists in coordinating the completion and accuracy of the individual resident’s care plan and the Medicare A coverage and documentation.

He/she must follow the documented care plan, accurately transcribe physician orders, and record all resident care. RN/LPN must maintain a Colorado RN or LPN license and must report any injuries or unsafe conditions to management immediately. Prepares Quality indicator summaries, and Quality Measures summaries.

Schedule, track and assure completion of MDS and documentation timely, submit complete and accurate MDS to State agency, conducts triple check on Med A claims in conjunction with office and medical records, and follows safety, infection control, and confidentiality standards. 

Qualifications:

  • AN or BSN is required
  • Current Colorado RN license required
  • Experience in MDS and regulatory standards for clinical, MDS, infection control, and care planning
  • Self-motivated and directed; excellent time management skills
  • Detail-oriented, ability to work with families, other departments, and agencies

Title CNA – Transportation and Central Supply

Position Overview:  This position assists with resident appointments, stocking and delivering supplies, works as a CNA on the floor, and participates in on-call rotation.

Key Duties & Responsibilities

Central supply duties include stocking and organizing the supply room, delivering supplies, checking expiration dates, doing mattress checks, and engraving residents’ hearing aids, dentures, etc.

Transportation Duties include making resident appointments, coordinating notices and transportation, and transporting residents, ensuring safety.

CNA duties include working on the floor, providing assistance to residents, following all policies and procedures of Mount St Francis.

On-Call responsibilities are every other weekend, and include taking calls after normal work hours regarding call-offs and scheduling issues, and keeping accurate records of times and activities.

Other duties include assisting Staffing Coordinator/DON/NHA with clerical duties, as assigned.

Qualifications:

  • Current CNA license in good standing and CPR
  • At least one year related experience in a health care setting

Title Nutrition Aide – Part Time

Position Overview: Under the direction of the Nutrition Supervisor, the Nutrition Aide is responsible for the set-up, service of meals, and clean-up of dining area for the residents, to include sanitation of the area as assigned and according to all organizational policies and procedures.  Prepare dining room, set and buss tables, clean salt and pepper shakers, assure food is hot and plates are attractively assembled, prepare sandwiches, soups, and beverages in the galley, and apportion food servings in accordance with dietary lists. Collect dishes, trays, and trash, clean and disinfect work areas, tables, and cabinets.  Prepare room trays, as needed, and assure snacks are available and galley cabinets are appropriately stocked.

 Qualifications:

    • Must be at least 18 years of age
    • Ability to multi-task, be mature, responsible and a team player.

    • Ability to regularly lift/move up to 25 pounds, frequently lift/move up to 50  pounds and push/pull up to 100 pounds.
    • 2 years experience working with senior population, along with experience working in healthcare food service is preferred.
