Position Overview: The Certified Nursing Assistant (CNA) is responsible for direct care of nursing center residents/patients.  Typical  responsibilities include answering call lights, bells, or intercoms to determine resident needs, bathing, dressing and undressing residents, serving food, transporting residents, assisting residents to walk, changing bed linens, taking and recording temperature, blood pressure, pulse, and respiration, and providing direct nursing care according to all organizational policies and procedures.

Key Duties & Responsibilities:

People: The CNA will work as a member of our team ensuring the timely, compassionate, and appropriate care of residents.  He/she will communicate effectively with co-workers and the Nurse in Charge.

Quality: The CNA must adhere to all safety and procedural requirements in order to provide quality care.  He/she must follow the documented care plan, perform correct transfers of residents, understand and follow correct equipment operations and activities, provide ROM/positioning and ambulation of residents, and assist in providing the least restrictive environment for residents. Must maintain a Colorado CNA license.

Operational Excellence:  The CNA will participate in care planning, admissions and discharge processes, and orientation of new CNAs. The CNA will be dependable and reliable in attendance.   

Qualifications:

    • Must have current CNA license
    • Must be at least 18 years of age
    • Experience in long term care as a CNA is preferred
    • Experience with dementia is a plus

Schedule:  Full-time averages 32 – 37.5 hours per week.   Part-time averages 15 – 20 hours per week. Shifts are days (6 AM – 2 PM), evenings (2 PM – 10 PM), nights (10 PM – 6 AM) including some weekends.

Position Overview: Under the direction of the Activities Director, will assist in conducting life enhancement activities in the nursing center.

Key Duties & Responsibilities: The Activities Assistant prepares for and leads therapeutic activities for residents, assists in transporting residents, coordinates volunteers, and maintains records.

People:  The Activities Assistant demonstrates the Mount St. Francis ministries’ mission and values in dealing with residents, family members, and other staff, effectively leads resident activities, and transports residents to and from activities, to include driving the activities bus.

Quality:  Assistant supports quality in the Activities Department by preparation of activities areas for scheduled events, keeping attendance records, and monitoring volunteers.

Operational Excellence:  Monitors activities/events to ensure a therapeutic environment and provides necessary assistance to residents.

Qualifications: CNA license is preferred but no required. Must be able to perform each essential duty of the job, have a high school diploma or GED, have one to three months related experience, or an equivalent combination of education and experience. Requires a valid Colorado driver’s license.

Schedule: –Approximately 17.5 hours per week, Tuesdays, Thursdays, and every other weekend.

Position Overview: Under the supervision of the Director of Nursing, the Clinical Nurse Manager implements nursing care procedures, regulatory guidelines, and nurse practice protocols through education, communication and assessment of individual patient needs. The Clinical Nurse Manager performs supervisory responsibilities, to include planning, assigning, directing, and monitoring work of assigned staff.

Quality: The Clinical Nurse Manager will monitor recommendations of QI committees for compliance, work as a liaison with nursing management, consult with the DON on nursing problems and interpretation of regulatory guidelines, policies and procedures, implement policies and procedures approved by the DON and Executive Director of Health Services, oversee the infection control program, adhere to all safety and procedural requirements in order to provide quality care. Must maintain a Colorado RN license and must report any injuries or unsafe conditions to management immediately.

Operational Excellence: The Clinical Nurse Manager assures compliance of all standards in the unit, works closely with other departments to meet the needs of the residents, participates in on-call staffing rotation, monitors 24 hour report daily for clinical issues, assures Coumadin, blood sugar, and insulin tracking is complete and accurate, and audits new admission orders and assessments.  

Qualifications:

    • Must have current RN license in good standing and CPR certification
    • AN or BSN is required
    • Must be at least 18 years of age
    • Minimum five years experience in long term care nursing
    • Ability to stand and walk frequently
    • Ability to lift or move up to 50 pounds and appropriately use mechanical lifts
    • Experience with dementia is a plus

Schedule: Average 40 hours per week including every other weekend and some holidays, including participation in on-call rotation. May exceed 40 hours per week, as needed.

Position Overview:   As a resident advocate, counsels and assists nursing center residents and families needing assistance.  Contacts appropriate agencies such as ombudsman, adult protection agency, psychiatrists and psychologists, as necessary.  Provides interventions to meet psychosocial needs of residents and works with the Nursing Center team in setting resident goals. Understands the Patient Bill of Rights and ensures rights are not violated.

People: Provides consultation to staff, community agencies, etc., in efforts to solve the needs and problems of the residents.  Assists residents with problems such as personal and family adjustments, finances, food, clothing, and physical and mental impairments.  Provides information to residents and families on advanced directives, living wills, and limited medical treatment.  Investigates allegations of abuse, documents findings and recommends action to be taken.

Quality: Assists with discharge planning in order to ensure a smooth transition to home.  Performs and documents quarterly and annual assessments.  Secures information about medical, psychological, and social factors contributing to resident’s situation and evaluates.  Works with medical and nursing teams regarding appropriate use of psychotropic medications and documentation of behavioral patterns. Participates in care conferences as social services member of interdisciplinary care team. Recommends interventions of other health professionals such as nursing or physician consults.

Operational Excellence:  Keeps abreast of current Federal and State Regulations, as well as professional standards and makes recommendations for changes in policies and procedures.  Coordinates admissions in absence of Admissions Director.  

Qualifications: 

  • B.S. in Social Work or BA in Human Services or one to two years related experience and/or training with consultation from MSW.
  • Previous LTC experience required.

Assure the accuracy of the MDS through communication and documentation. Assists in coordinating the completion and accuracy of the individual resident’s care plan and the Medicare A coverage and documentation.

Schedule, track and assure completion of MDS and documentation timely, submit complete and accurate MDS to State agency, conducts triple check on Med A claims in conjunction with office and medical records, and follows safety, infection control, and confidentiality standards.

  • AN or BSN is required
  • Current Colorado RN license required
